One of the major uses of Juvaderm is in the filling of nasolabial folds. Nasolabial folds are the deep folds that run between the sides of the nose to the corners of the mouth, and in older people are continued onward as mannequin lines. These natural folds which in the young are shallow linear depressions, with aging and gravity become deeper and more pronounced as the skin loses its volume and turgidity and supporting dermis and gravity drags the cheek pad downward toward the ground. The deepening of these folds makes a person look very much older. It is thus imperative that a person who wants to look young do something to her nasolabial folds as soon as possible.

Nasolabial folds can be treated with the injection of face fillers, one of which is Juvaderm, also commonly misspelt as Juviderm. Juvaderm or Juviderm is a cross-linked hyaluronic acid filler in an injectable gel form. Hyaluronic acid is a natural component of the dermis of the skin, and its injection into the skin will allow the skin to regain its volume and turgidity as well as allow absorption of water. The nasolabial fold thus becomes shallower and less obvious. The cross-linkage of the hyaluronic acid in Juvaderm allows the substance to last up to one year before being absorbed into the body. Thus the correction of the nasolabial fold achieved will remain for about 1 year.

The procedure is simple. At the initial consultation, the patient’s face is assessed to see the extent of the fold, and to assess how much Juvaderm is needed. General health is assessed to ensure the patient can tolerate the injection, and that there will be no complications due to the injection such as excessive bleeding in persons with bleeding disorders. The procedure is usually done under local anesthetic.

One hour before the procedure, liberal amounts of local anesthetic gel are applied to the nasolabial fold, and an occlusive plastic wrap is applied to the gel to aid local skin absorption of the anesthetic gel. Some doctors prefer to give a local anesthetic injection to the nerve supply of the cheek (the infraorbital nerve) through the mucosa of the mouth to ensure better anesthesia.

When sufficient anesthesia has been obtained, the patient is placed either in a sitting or semi-reclining position. The area is cleaned with spirit, and the syringe of Juvaderm gel is fitted with usually a 27G 1/2inch needle. The contents of the syringe are emptied in multiple unbroken lines extending all the way from the hollow next to the nostril to the corners of the mouth. After this the needle is withdrawn, and the injected gel massaged and pressed to ensure esthetically pleasing results. This injection is usually bloodless and the patient is seen again in 1 week for reassessment and top-up of Juvaderm, if necessary.

The following video shows a board certified surgeon injecting nasolabial folds in a model with Juvaderm. Juvaderm is a smooth-flowing hydrophilic cross-linked hyaluronic acid filler. Its major advantage is that it lasts longer (up to 1 year), and its physical properties allow it to be injected more easily and in a more shallow plane than other fillers. Its hydrophilic nature allows it to absorb water over time and smoothen out any minor defects in the injection technique.

This video also shows the placement of Juvaderm just below the corners of the mouth to correct the minor downward drift of the corners with age, so that a more youthful mouth is created.

The story of a typical Juvaderm candidate: We have to age. Fact of life. And when we do, the skin changes. As we age the natural volume of the younger skin starts to diminish, and wrinkles and folds form. Added to this is the effect of gravity. As we get older, gravity tends to drag the structures on the face lower, eyebrows become heavier and it takes a greater effort to keep our eyes open, the skin over the cheekbones loosens and becomes heavier resulting in deep nasolabial folds (the fold running from the sides of the nose to the corners of the mouth), and the jowls become heavier, resulting in a bulldog look.

Unfortunately, much as we would like to tout Juvaderm (or more correctly known as Juvederm by Allergan) as the answer to the above, we have to be realistic. Juvaderm is a filler, not an antigravity agent. Thus we need to know that Juvaderm will play a very useful role in eliminating the earlier nasolabial folds which are not too deep (let your doctor see you to determine if you fall into this category). When the effects of gravity become too severe, it is time for a facelift, or if things are not too severe yet, one can use what are commonly known as threadlifts, where the overhanging skin is lifted up against gravity by a thread running under the skin).

Back to Juvaderm. Scientifically, this product is described as a crosslinked hyaluronic acid that comes in the form of a smooth clear gel. Hyaluronic acid is a substance that occurs naturally in the skin that helps to maintain the volume and hydration of the skin. The crosslinkage in the gel helps to slow down the natural breakdown of hyaluronic acid in the body so that the effect in the body lasts longer. Juvaderm is injected by a skilled doctor to your face under the skin at the site of wrinkles in order to lift up the skin in that area. Skin volume is thus restored and wrinkles and folds disappear.

This injection procedure does not take more than a half hour and you can easily have this done during your lunch break and go back to the office after the break, effectively making this a lunchtime procedure. Of course before the procedure is done the doctor will have to see you and take a careful history and examination of your face and the troubled areas. Just before the procedure the doctor may elect to give you some anesthetic to minimise or eliminate any discomfort of the injection. This is either done by application of anesthetic creams, or local injections of anesthetic. Rarely is the procedure done under general anesthesia.

After the procedure you may be given painkillers to reduce any residual pains, or some medicine to reduce the minimal bruising which may occur as a result of the injection. Follow up is recommended a week after your Juvaderm filler injection to ensure the desired result is achieved.
